Select this option for an in-person consult with our doctor as required by the Mississippi Department of Health to apply for your 1 year Mississippi Medical Marijuana Card. As a first time Mississippi Medical Cannabis card holder, you are required to have an initial in-office visit with our doctor AND an additional follow-up visit in-office or via telemedicine at 6 months. Failure to complete the 6 month follow-up will result in a mandatory report to the Mississippi Department of Health with possible revocation of your card. This consult includes the following: 1. Your initial in-office visit with our doctor for your initial 12 month certification. 2. A FREE 6 month follow-up consult (in-person OR telemedicine) with our doctor as required for the remaining additional six months of certification. 3. Helping you obtain your medical records if you have not already acquired them. NOTE: WE CANNOT SEE YOU UNTIL WE RECEIVE YOUR MEDICAL RECORDS. Please complete our Patient Intake Form before booking. 4. Taking and providing a picture of yourself which is required by the Mississippi Department of Health when completing your Mississippi Medical Cannabis application. 5. Assisting with setting up your Mississippi Medical Cannabis Online Profile. If you have been previously diagnosed with one of the Current Qualifying Conditions, then we are here for you. If we find that you do not qualify for your Mississippi Medical Cannabis card, you will receive a full refund. You must be an Mississippi resident over 18 and NOT an ACTIVE member of the military in order to apply (Mississippi National Guard included). PLEASE NOTE: Before booking your appointment, please complete our Patient Intake Form where you will upload a picture of your valid Mississippi Driver's License (or valid State of Mississippi issued ID).
