If purchasing this service, you will need to book a follow up anointment within 6 months after seeing our doctor. An one on one in person consult with our doctor so you may receive the Physician Written Certification required to apply for your Mississippi Medical Marijuana Card. This consult includes the following: 1. Your initial in office with our doctor for your first 6 month certification. 2. Help obtaining your medical records, if you do not already have them. NOTE: IF WE ARE REQUESTING YOUR RECORDS WE CAN NOT SEE YOU UNTIL WE RECIEVE THEM FROM YOUR TREATING DR. Please complete our Patient Intake Form before booking. 3. We will take and provide you a picture of yourself that is required by the State when completing your MMJ application. 4. Assistances with setting up your MSMMJ Online Profile. If you have been previously diagnosed with one of the Current Qualifying Conditions, then we are here for you. If we find that you do not qualify for your MMJ card, then you will receive a full refund. Unlike, the big cooperate clinics, our re-certs are drastically cheaper than first time certifications. You must be an Mississippi resident over 18 and NOT an ACTIVE member of the military (Mississippi National Guard included). PLEASE NOTE: Before booking your appointment please complete our Patient Intake Form, where you will upload a picture or your valid Mississippi Drivers Licenses (or valid State of Mississippi issued ID), a picture of your previous MSMMJ card, and let us know if you will be bringing your medical records confirming your qualifying condition diagnosis or if we need to request them from your doctor. All needed documents may also be faxed or emailed to us.
